Terms & Conditions

This website (“Site” or “MyChart”) and related services are provided by The Medical College of Wisconsin, Inc. and/or Froedtert Health, Inc. affiliates: Froedtert Memorial Lutheran Hospital, Inc., Community Memorial Hospital of Menomonee Falls, Inc., St. Joseph's Community Hospital of West Bend, Inc., Froedtert & The Medical College of Wisconsin Community Physicians, Inc., West Bend Surgery Center, LLC, or Froedtert Surgery Center, LLC (“Health Care Providers”) to patients subject to compliance with the terms and conditions set forth below. Please read the following information carefully. Your continued use of MyChart will indicate your agreement to be bound by the terms and conditions set forth below. If you do not agree to be bound by these terms and conditions, promptly exit MyChart.

I. Privacy

Your privacy is of the utmost importance to us. Health Care Providers will use your confidential medical information in order to provide you health care services. Health Care Providers will, at all times, maintain the privacy of your confidential medical information available within MyChart and will not disclose such medical information about you unless you authorize the disclosure of your information or the disclosure of the information is permitted by law. Please review Health Care Providers Notice of Privacy Practices for a thorough description of how we gather, use and protect your confidential medical information. All messages sent and received within MyChart that contain medical information are subject to state and federal laws governing the security and confidentiality of medical records.

II. MyChart Features

A. Electronic Messaging

MyChart should never be used for urgent matters. The anticipated turnaround time for response to electronic messages is two business days. If you believe you are experiencing an urgent or life threatening health condition, please call your physician’s office, go to an emergency room of a local hospital or dial 911.

If your provider is out of the office or unavailable, messages sent via MyChart may be routed to other authorized caregivers within Health Care Providers in order to facilitate a timely response to your request.

Health Care Providers may send you messages via MyChart. It is your responsibility to monitor these messages and respond in an appropriate manner.

B. Secure Communications

All communications between you and Health Care Providers using MyChart occur over a secure connection. You are required to provide Health Care Providers a valid e-mail address and it is your responsibility to keep this address updated if you change your e-mail account. You will receive Internet e-mail messages notifying you that new information is available in your MyChart account. Although these e-mail messages will not contain any confidential medical information, the notification that new information is available by accessing MyChart may be information that you would not want others to know. Please take this into account when providing an e-mail address or using MyChart. All of the confidential medical information available to you in MyChart is protected and securely maintained by Health Care Providers.

C. Activation Code

When you initially enroll in MyChart , you will be provided with an activation code. The activation code you receive will give you the opportunity to create a personal login name (called MyChart ID) and a password. Once this activation code has been used it will no longer be valid. It is extremely important that you keep your login name and password completely confidential. Anyone with access to your login name and password will be able to access your confidential medical information. That person will also be able to read your messages and send new messages as if that person were you. It is your responsibility to prevent disclosure of your login and password, and to change your password if you feel that its security has been compromised. You may change your password at any time.

D. Features

At this time, Health Care Providers anticipates that the MyChart web portal will provide the following services:
* Ability to review, request or cancel your appointments
* Ability to view selected portions of your medical record such as allergies, medications, histories, problem list and recent visit information
* Ability to receive selected lab results
* Ability to communicate with your health care provider via secure messaging regarding non-urgent health concerns
* Ability to request prescription renewals
* Ability to link to other sites

III. Information Does Not Constitute Medical Advice

Health Care Providers assume no responsibility for how you use the information you obtain from this Site. You should consult with a physician or other health care provider regarding your own condition and how this Site content may or may not apply to you. You may be accessing your own medical information via the MyChart application and may wish to communicate with one of your health care providers about that or related information. If you have an emergency, or think you need to speak to someone urgently, please do not use the e-mail communication options provided to you by MyChart. Neither MyChart nor any other application available through this Site is a substitute for appropriate and timely contact or consultation with your physician. You should never change or stop any course of treatment prescribed by your physician without first consulting him or her.

IV. Termination of Access

Health Care Providers may restrict or terminate access to MyChart at any time. If access is restricted for planned maintenance of the MyChart system, reasonable efforts will be made to communicate downtime to users in advance. However, there may be reasons to restrict access to MyChart without advance notice. If your use of MyChart violates these Terms and Conditions, Health Care Providers may permanently terminate your access to MyChart.

V. Restrictions on Use and Ownership

All pages within this Site and any material made available for download are the property of Health Care Providers and/or its subsidiaries and affiliates. The content on this Site is provided for Health Care Providers and their patients only. The names of the Health Care Providers and their affiliates, including, without limitation, the names Medical College of Wisconsin, Froedtert Hospital, Community Memorial Hospital, St. Joseph's Hospital, Froedtert & Medical College of Wisconsin Community Physicians, West Bend Surgery Center and Froedtert Surgery Center, and their associated corporate logos, are the trademarks and service marks of The Medical College of Wisconsin, Inc. and Froedtert Health, Inc. and their affiliates. All other trademarks, service marks, and logos used in this Site, including but not limited to MyChart and Epic, are the trademarks, service marks, or logos of their respective owners. Nothing on this Site should be construed as granting any license or right to use any trademark, service mark, or logo displayed on this Site without the express permission of Health Care Providers or the third party that may own the trademark, service mark, or logo.

VI. Accuracy and Integrity of Information

Although Health Care Providers attempt to ensure the integrity of this Site, it makes no guarantees whatsoever as to the correctness or accuracy of this Site. It is possible that this Site could include typographical errors, inaccuracies or other errors. In the event that an inaccuracy is identified, you should inform Health Care Providers so that it can be corrected. Information contained on this Site may be changed or updated without notice.

VII. Availability

MyChart may not always be available to you due to system maintenance, back-ups or failures. Access is provided on an "as is available" basis and you are not guaranteed access at any particular time.

VIII. Confidentiality

Both Federal and Wisconsin laws govern the confidentiality of patient health care records. Health Care Providers are dedicated to keeping your medical information available within MyChart confidential. Our employees are subject to strict standards for maintaining the confidentiality of your medical information. By allowing you access to your information, you must share in this responsibility.

IX. Security

The Site places considerable emphasis on the privacy of medical information and compliance with State and Federal privacy laws. MyChart is configured to be secure from unauthorized access, but Health Care Providers and this Site cannot be held responsible for:
* Absolute security of all electronic communication transmissions between patient and Health Care Providers;
* Unauthorized disclosure resulting from a user not logging out of an active session;
* Unauthorized disclosure resulting from a lost or stolen user ID and password;
* Unauthorized disclosure resulting from information printed by the user from MyChart;
* Unauthorized disclosure resulting from personal computer settings or installed software products that may compromise information security.
* Similar events outside the substantial control of Health Care Providers or the Site.

X. Links to Other Sites

Health Care Providers may create links on this Site to other websites. Health Care Providers make no representations whatsoever about any other website that you may access through this Site or any entity that may post its advertisements on this Site. When you use the links on this Site to access third party websites, those websites are independent from this Site and Health Care Providers have no control over the content on such websites or the privacy policies of such websites. Health Care Providers do not endorse or accept any responsibility for the content or use of any linked site. If you decide to access any third party website linked to this Site, you will be leaving this Site and you do so entirely at your own risk. You will not necessarily be notified by this Site when you are leaving this Site. We would like to hear from you if you find any websites linked to or from this Site objectionable.

XI. Your Right To Object

If you believe Health Care Provider(s) is using your personal identity information in a way that is not consistent with the Terms and Conditions, you should contact us at once.
